Heidi Lowe and Client Coaching Agreement

 This Coaching Agreement confirms that the fee for a 50-minute coaching session with Heidi Lowe is $99. Coaching will occur on a month-to-month basis and in the event that either party decides to cancel further services, the client and coach agree to provide one another with thirty days notice.

The service to be provided is coaching, which is not advice, therapy, or counseling. It may address specific personal projects, a business venture or general conditions in the client’s life or profession. Coaching is designed to be an alliance that helps bring out the best in the client and helps the client create and live the life they want to live.

The coach guarantees that all information provided to the coach by the client will be kept strictly confidential as allowed by law. Throughout this working relationship, the coach will engage in direct and personal conversation with the client. The client can count on the coach to be honest and straightforward in asking questions and making requests. The client understands that the power and effectiveness of the coaching relationship are dependent on the client. If the client believes that coaching is not working as desired, the client agrees to immediately communicate that to the coach and both parties will take action to remedy the situation. There are no refunds if the client deems the coaching sessions as ineffective or doesn’t meet with their expectations. There will be no refunds for sessions missed by a client unless 24 hours or more notice is provided to the coach. 

I, the client, hereby certify that I do not suffer from any physical or mental disability or illness that may affect my participation in the coaching process, and/or if I have any substance abuse problems or mental illness, I have consulted with my physician and/or health care professional and I have been advised that I may participate in the coaching process without injury or risk. I agree that if there is any change in this representation, I will immediately advise the coach.

Purchase of any coaching session(s) is confirmation of my full understanding of and agreement with the terms and conditions that are outlined above.
